For cases which delayed neurological improvement i.e more than 2 weeks, when ossification has stared to take place, how would you suggest the case be approached?
For the peek implant for cranial and frontal bone defect, how long after initial surgery can the implants be placed?
Surgical timing is primarily dictated by the management of life-threatening injuries. If a patient lacks anesthesia clearance due to cardiopulmonary instability or the risk of brain herniation from severe swelling, surgery must be deferred—even if the bone has begun to fuse. In cases of gross displacement, bone fragments are rarely close enough to fuse prematurely. Patient safety must always precede facial fracture repair to minimize postoperative complications.
Regarding brain swelling, the risk of herniation is significantly reduced if a cranial vault bone flap has been left out. In a worst-case scenario where immature fusion has begun, a controlled osteotomy can be performed; a light tap with an osteotome will typically split the bone along the new fusion line.
Success in these complex cases is often hospital-dependent and relies heavily on the quality of specialist support. I am fortunate to work with an exceptional neurosurgical and anesthesia team accustomed to these high-stakes procedures.
For PEEK implants, I recommend waiting 6 to 12 months to allow all soft tissue swelling to subside. The skin is ready when it begins to slide freely over the underlying structures. It is critical to raise a thick skin flap to prevent implant extrusion; ideally, the dura should not be exposed. I apologize that the cranioplasty section of the atlas is still being edited.
