Incision is made around inferior aspect of umbilicus
Superficial layer of Fat pad is located in the subcutaneous tissue plane
Grab the fat graft with a clamp to lift it away from the surgical field
Deep plane is the rectus muscle facia. If you stay on top of this fascia, you will be safe! While lifting the fat graft, use bovie or scissors to harvest fat graft.
For relatively small abdominal fat graft harvest as seen here, drain is not necessary. Close with 3-0 vicryl buried suture, 4-0 or 5-0 Fast Absorbing Gut sutures for skin.
For large composite abdominal fat graft harvest, drain may be necessary. Furthermore, local tissue advancement of surrounding abdominal fat pad may be necessary to avoid severe contour irregularity. Such case will be covered in a separate surgical video.
